Is the sagittal configuration of the cervical spine changed in women with chronic whiplash syndrome? A comparative computer-assisted radiographic assessment
(2002) Kristjansson, E.; Jónsson, H.; Department of Engineering
Objective: To reveal whether women with chronic whiplash-associated disorder (WAD) symptoms, grade I-II, demonstrate regional and/or segmental radiographic signs of altered cervical lordosis. Design: Case-control study. Setting: Radiography department at a university hospital. Participants: Three age-balanced groups comprising 120 women. The case group included women with chronic whiplash syndrome (n = 41), and the control group included women with chronic insidious onset neck pain (n = 39) and an asymptomatic group (n = 40), who were given baseline data. The sample was referred from informed doctors and physiotherapists. Intervention. The women sat in a standardized sitting position and radiographs were taken in a lateral position with fluoroscopic control for alignment. Outcome Measures: Two distinct measurements were taken; 1 of the angles of the upper and lower cervical curvatures, respectively, and 1 of the angles between the inferior borders of each pair of vertebrae in the lower cervical spine. The 3 groups were compared on the ratio of the lower to upper cervical spine angles and on the mean angular values for each segment in the cervical spine. Results: The whiplash group showed a decreased ratio between the lower versus upper cervical spine but comparisons between groups were not statistically significant. The whiplash group was in a significantly more flexed position at the C4-C5 level compared with the asymptomatic group (P = .007). The reliability measures have to be strengthened to render these results definitely conclusive. Conclusion: The whiplash group exhibited a different configuration of cervical lordosis. This is clinically important and needs to be studied more closely.

Incidence of total hip replacement for primary osteoarthrosis in Iceland 1982-1996
(1999) Ingvarsson, Thorvaldur; Hägglund, Gunnar; Jónsson, Halldór; Lohmander, L. Stefan; Department of Engineering
We report the incidence of total hip replacements performed in Iceland between 1982 and 1996. During this period, 3403 hip arthroplasties were done. The annual number of procedures increased from 94 hips in 1982 to 323 hips in 1996. Annual rates of total hip replacements due to primary osteoarthrosis per 105 inhabitants were 68 in 1982-1986, 90 in 1987-1991, and 114 in 1992-1996. In the years 1992-1996, the age-standardized incidence of total hip replacements for primary osteoarthrosis was 3/105 among patients younger than 39 years of age, while it was 621/105 among those 70-79 years of age. The mean age at surgery for primary osteoarthrosis was 69 years in both men and women. Incidence rates in various countries are difficult to compare, but by using age-standardized data and correction for differences in population structures between Iceland and Sweden, we find that the incidence of total hip replacement for primary osteoarthrosis of the hip is at least 50% higher in Iceland than in Sweden. This difference is consistent with the higher prevalence of hip osteoarthrosis observed in Iceland than in Sweden.

Embolization of spinal metastases reduces peroperative blood loss : 21 patients operated on for renal cell carcinoma
(1993) Olerud, Claes; Jónsson, Halldór; Löfberg, Ann Marie; Lörelius, Lars Erik; Sjöström, Lennart; Department of Engineering
We evaluated the peroperative blood loss in 21 patients who were treated with 29 operative procedures for thoracolumbar spinal renal cell carcinoma metastases. Embolization was undertaken prior to 11 operations. At posterior surgery the average blood loss was one third after embolization compared with when embolization had not been performed. the blood loss was also lower during anterior surgery, although this difference was smaller. We recommend that preoperative embolization should precede surgical treatment of spinal metastases of renal cell carcinoma where it can be anticipated that the operation will extend into the pathological tissue.

Surgery for multiple myeloma of the spine : A retrospective analysis of 12 patients
(1992) Jonsson, Bogi; Sjöström, Lennart; Jónsson, Halldór; Karlström, Göran; Department of Engineering
We report on 12 surgically treated patients with myeloma of the spine. Pain relief was obtained in all the patients. Paraparesis was present in 5 patients. All 12 patients subsequently became ambulatory. the average postoperative survival was 2.3 years. Eight patients, all without symptoms from the treated lesion, survived for more than 1 year. Our experience suggests that surgical treatment of multiple myeloma of the spine may be rewarding.

Posterior reduction of a pathologic spinal fracture a case of indirect anterior Dural decompression
(1992) Olerud, Claes; Sjöström, Lennart; Jónsson, Halldór; Karlström, Göran; Department of Engineering
In a patient with paraparesis resulting from a pathologic fracture of Th9, which was due to a breast cancer metastasis, indirect anterior decompression via posterior reduction was performed. the rationale for the operation technique was the similarity between a pathologic fracture of the thoracolumbar spine and a traumatic burst fracture. the method, which uses a transpedicular reduction and fixation device, is substantially less invasive than a formal anterior decompression.
